...then write an episode around that clever title? (since most of the
episode titles are now plays on popular movies or such).
Or did they just happen to write an episode about Homer getting
attacked by a bear, and came up with "The Fat and the Furriest".

I'm pretty sure they write the episode first and then come up with a
clever title later. Most of the DVD commentaries seem to support this.
A writer comes up with an idea, pitches it at a meeting, is assigned to
write the script, and others punch it up with some funnier jokes.
Somewhere in this process somebody comes up with a clever title that
everyone likes.
