Obi-Wan's planet of birth is not known. Many sources will say he's from
Tatooine. That's partly because he lived there so long and because
before the Prequel Trilogies Owen was actually Obi-Wan's brother, not
Anakin's step-brother.
As for the planet Obi-Wan was raised on, that would be Coruscant in the
Jedi Temple. All Jedi training starts when they're babies (with a
couple of exceptions) and they don't usually ever know their birth
families.

Palpatine is from Naboo, but he was also the Senator for a few planets
in the same sector. BUT Palpatine is known to have altered his records,
so he may not actually be from Naboo at all. :-)

Being human in the Star Wars universe does not necessarily mean that you are
from Earth, it means being humanoid.

True. I forgot that fact. Everything is possible in Star Wars.
